  • Log in to the UHC Provider Portal to access patient claim details.
  • Search for the Claim Status of the specific patient.
  • Scroll to the ACTION Tab and click “VIEW OR ACT ON YOUR CLAIM.”
  • Navigate to the ASSESSMENT Tab and select “CREATE CLAIM RECONSIDERATION.”
  • Enter or select all relevant details and attach supporting documents (e.g., HCFA form).
  • Click SUBMIT to receive a Ticket Number for future reference.
  • Go to the MY APPS section, select UHC Claims Reconsideration, and enter provider details with the Ticket Number to track your request.
