- Log in to the UHC Provider Portal to access patient claim details.
- Search for the Claim Status of the specific patient.
- Scroll to the ACTION Tab and click “VIEW OR ACT ON YOUR CLAIM.”
- Navigate to the ASSESSMENT Tab and select “CREATE CLAIM RECONSIDERATION.”
- Enter or select all relevant details and attach supporting documents (e.g., HCFA form).
- Click SUBMIT to receive a Ticket Number for future reference.
- Go to the MY APPS section, select UHC Claims Reconsideration, and enter provider details with the Ticket Number to track your request.
- This streamlined process helps practice managers, healthcare providers, and owners efficiently manage UHC claim reconsiderations.
